The core function of Carbal Medical Services is to provide medical services to Aboriginal & Torres Strait Islander people through two medical practices and multiple community programs and initiatives. Carbal covers the regions of Darling Downs, Southern Downs and Goondiwindi.
Position Overview
The purpose of the NexiCare Home Care Worker role is to provide high-quality, person-centred care to participants under Aged Care Programs such as the Commonwealth Home Support Program (CHSP), the Home Care Packages Program (HCP), and the Short-term Restorative Care Program (STRC), and when implemented in 2025, the Support@Home Program.
The role involves delivering tailored support services that align with clients’ individual needs and preferences, while continuously assessing and identifying areas of risk to ensure proactive care.
This position is also responsible for ensuring that all services comply with Aged Care legislation and quality standards, contributing to the overall well-being, independence, and quality of life of the clients.
Provide Services under programs:
- Commonwealth Home Support Program
- Home Care Packages
- Short Term Restorative Care Program
- Support at Home Program (from July 2025)
Deliver services including but not limited to:
- Unaccompanied Shopping
- Accompanied Activities, e.g. Shopping, social activities, etc
- Assistance with Self Care
- Assistance with Client Self-administration of Medicine
- Social Support; Individual and Group
- Domestic Assistance
- Home and garden maintenance
- In-home respite
- Transportation
Ensure the delivery of culturally safe, client-centred aged care services of the highest quality, in accordance with client care plans, and in compliance with the Charter of Aged Care Rights, and the Code of Conduct for Aged Care.
Comply with rostering systems to ensure efficient service delivery.
Utilise client software system; Visual Care in understanding and contributing to identified clients’ goals.
Accurate and timely documentation of service delivery.
Identify and report hazards and risks.
Build strong rapport and trusted relationships with clients, their families, and their representatives.
Support clients to provide feedback and make complaints.
Recognise and report changes or deterioration in the client’s physical, emotional and cognitive condition.
Respond to and report client incidents and participate in the investigation of incidents.
Provide feedback to Care Partners as required to inform care planning.
Contribute to governance and continuous improvement processes, and ongoing risk mitigation strategies.
Participate in quality and compliance activities associated with the Aged Care Quality and Safety Commission as required.

Selection Criteria
Essential
- ABN
- Certificate III in Individual Support/Ageing
- Have an understanding or the ability to rapidly acquire an understanding of the Aged Care Quality Standards and ability to work with clients with various levels of aged care services access requirements.
- Integrity, honesty, strong work ethic and demonstrated ability to work as part of a team.
- The ability to keep an accurate record of supports provided
- A C Class driver’s license with a Reliable motor vehicle with comprehensive insurance
- Hold or be eligible to obtain a Blue Card
- Hold or be eligible to obtain a National Police Check
- Current First Aid/ CPR
Desirable
- Identify as a person of Aboriginal or Torres Strait Islander descent
- Previous experience in the NFP, aged care or disability sectors
- The ability to understand and implement individual care plans
- Experience in working effectively with Aboriginal and Torres Strait Islander people and an understanding of Aboriginal and Torres Strait Islander Culture.
- Immediate start preferable

The core function of Carbal Medical Services is to provide medical services to Aboriginal & Torres Strait Islander people through two medical practices and many community programs. Carbal covers the regions of Darling Downs, Southern Downs and Goondiwindi. In addition to this organisational remit, Carbal provides NDIS services to community members throughout the region.
Position Overview
Carbal is seeking support workers to provide exceptional care and support for some of our most vulnerable people in their homes, and in the community. If you are keen to learn, like to help people, have your own vehicle and a mobile phone we would like to meet with you.
Key Responsibilities
- Be part of a multidisciplinary team addressing mental health in the community;
- Work one-on-one with clients within their homes and assist them to access the community;
- Be driven, motivated and have a passion to assist clients to foster independence and resilience and achieve their goals;
- Possess high level communication skills, the ability to work under supervision and a thirst to learn;
- Work closely with GPs, Aboriginal Health Workers, other health professionals and other NDIS Providers;

You will work alongside other reception staff and be responsible for ensuring all aspects of reception are covered. Ideally you will be familiar with the medical software program “Best Practice” and possess a high level knowledge of Medicare systems and item numbers, especially in relation to Indigenous health. Naturally, experience in providing culturally safe services will be highly regarded and your sense of privacy and confidentiality will make you stand apart from other applicants.

Covering the Darling Downs and Southern Downs regions including Goondiwindi, this position will be based in our Toowoomba General Practice clinic and reports directly to the Health Practitioner Manager.
Position Overview
This role will see you working as part of a nursing team to deliver exceptional standards of care to Aboriginal & Torres Strait Islanders and you will be motivated and have a genuine desire to Close the Gap in Indigenous health outcomes.
Your brief is broad and extends across all aspects of the organisation. Your skills as a Nurse will ensure smooth and efficient function of the practice in a clinical setting. You will have an understanding of chronic disease issues affecting Aboriginal and Torres Strait Islander people and knowledge of Medicare entitlements for Indigenous clients in order to help the organisation meet its operational requirements, or the capacity to learn these quickly. You will also have knowledge of the national privacy principles and the importance of confidentiality to allow you to stand apart from other applicants
You will be supported directly by the Practice Manager, along with the multidisciplinary team within the clinic, and the other health programs within Carbal.

Requirements:
Essential
- Current AHPRA Registration as a Registered Nurse or an Enrolled Nurse.
- Demonstrate experience in delivering Primary Health Services. Ideally your experience will be in a General Practice and/or hospital setting or an equivalent combination of relevant experience and post education and training.
- Demonstrated ability to be proactive and work cohesively with Management to achieve patient outcomes.
- Demonstrated experience in working effectively with Aboriginal and Torres Strait Islander people and a clear understanding of Aboriginal and Torres Strait Islander culture, or the ability to rapidly acquire the same.
- Demonstrated ability to effectively self-manage work flow and report on outcomes against the action plan.
- Current Open C Class Driver’s License valid in Queensland.
